  • General Health/Medical Records

    To access your health records, you will need to complete a Release of Information form. Please stop by Whitworth University Health Services to obtain a form or contact us by email.

    Allow 5-7 business days to receive your health records after submitting your Release of Information form.

  • Immunization Records

    To access your immunization records, please log in to the health services online student health portal using your Whitworth network username and password.

    Online Student Health Portal

    Click on the My Profile link at the top of the page. Select Immun. History from the dropdown menu. Open your immunization history PDF document to view and print.
