  • Bias Incident, Harassment and/or Assault Reporting form – This form is intended for reporting student-related incidents. Other incidents may be reported to Ariane Mondragón Oglesbee, chief human resources officer (509.777.4320) and/or the appropriate area vice president.   • Direct Deposit for Reimbursements
  • Early Alert (aka Student Concern Form) – Use this form to notify the Student Success Team when you notice residents displaying concerning behavior (sudden mood shifts, health concerns, changes in eating or sleeping habits, etc.). This is NOT where you report incidents; use the Incident Report Form to document policy violations.
  • Event Request Form (formerly Facilities Request Form/FRF) – If you're putting on a program, you need to reserve your space and request catering and AV needs through this form.
  • Fire Safety – Restrictions and guidelines for residents regarding fire safety in the residence halls.
  • Guest Policy and Form – Residents are required to notify the university of their overnight guests.
  • Housing Lottery Information – Everything you could ever want to know about the housing lottery.
  • Help-a-Pirate Program – Assist a fellow Pirate who could use help with meals or other basic life/academic costs.
  • Incident Report Form – Use this form for any incident that requires documentation (e.g., policy violations).
  • Infosilem – Schedule a room on campus.
  • The Print Shop will handle all your big print jobs.
  • Program Evals – Now that your program is over, it's time to evaluate!
  • RA Reimbursement Request Form – Want to get reimbursed for a program or other RA expense? This is your only way.
  • Roommate Agreement – While this online version exists, for fall 2020 through spring 2021, residence life will be using printed copies, with RAs meeting directly with their residents to sign off on the agreement. Please consult your AC or AAC for more information.
  • Title IX Report Form – Use this form to report an incident involving an alleged violation of Whitworth University's policy against violent and destructive behavior focused on sexual misconduct (including sexual harassment, sexual assault, rape, gender-based harassment, domestic violence, dating violence, sexual exploitation and stalking).
  • Student Activity Liability Waiver Form – If you're taking students off campus for a program, every student needs to sign this.
