Student Recommendation Release Form

By completing this form you are giving the Whitworth University faculty and/or staff members that you designate below permission to include non-directory information in a letter of recommendation or on-line form for an official of another organization. You may list any number of faculty and/or staff names and e-mails below. This form may be completed multiple times if needed. The completed form will then be added to your educational record. This consent will remain in effect (even after graduation or separation from the university) until you revoke it by notifying the Registrar’s Office in writing. Any questions or concerns should be referred to the Registrar’s Office.
